A faithful woman named Maria dei Quercio prayed fervently to the Blessed Virgin, promising a public act of thanksgiving if her family was spared from the deadly disease. Mary heard her prayer, and her entire family remained untouched by the epidemic.

In gratitude, Maria commissioned a beautiful image of the Blessed Mother, accompanied by Saint Sebastian and Saint Roch, saints long invoked against plague, illness, and suffering. Around Mary's head were inscribed the angel's greeting, "Ave gratia plena" ("Hail, full of grace"), and above her the biblical title "Hortus Conclusus"—"Enclosed Garden." Drawn from the Song of Songs, the enclosed garden has long been a cherished symbol of the Blessed Virgin Mary's spotless purity, her perpetual virginity, and the sacred place she holds in salvation history.

The painting was placed on a wall beside the beautiful public gardens that stretched from the Captain's Palace toward the sea, where travelers, pilgrims, and townspeople could stop to pray and give thanks. Because of its location beside the garden wall, the image soon became affectionately known as Madonna dell'Orto—Our Lady of the Garden. Through the centuries, countless faithful have sought her maternal intercession, especially in times of illness, hardship, and uncertainty.
